Your Role with the Company :

• Full-time position flexible hybrid schedule working remote and on-site
• Clinical program manager is responsible for managing coordination for all operational and clinical activities for the Pediatric Concurrent Care, Palliative and Transitional Care programs
• Communicating with patients, families, physicians, community partners and interdisciplinary team
• Oversight of departmental budget, billing process & reimbursement, managing and supporting staff and representation of the programs in the community and internally

Background Profile:

• RN or NP with experience in pediatric hospice care
• Strong leadership experience
